Substance Use Treatment Program

We serve adults from all backgrounds and ethnicities to help them learn how to lead lives free of substance use. The Outpatient SUD Program is designed to assist Rocking Horse Community Health Center patients overcome the chronic disease of addiction. We approach each situation without judgment but rather with empathy and understanding. We will assist you to achieve a sustainable long-term recovery from the disease of addiction, and to build a healthy happy life.

Medication Assisted Treatment or M.A.T. is the use of medications in combination with counseling and behavioral therapies for the treatment of substance use disorders.

A combination of medication and behavioral therapies has been shown to increase the effectiveness of treatment of substance use disorders, and can help people sustain recovery.

SUD services provides

  • Outpatient SUD Individual Therapy (weekly, biweekly, and monthly)
  • Group Therapy (Mornings and Afternoons, food/snacks provided, attend a minimum of 12 groups to receive a $50 dollar gift card, and a Recovery coin)
  • Primary Care Physician/Nurse visits.
  • MAT (Schedule with PCP after assessment completed, buprenorphine (Suboxone, Subutex, Sublocade) and naltrexone (Vivitrol)
  • Harm Reduction (determined by PCP, Therapist)
  • Psychiatric Mental Health Nurse Practitioner consultation if needed.
  • SUD Care Coordinators
  • After Care Services (link with services, employment, housing)
