Family Practice Medical Home

Since March of 2008, Rocking Horse Community Health Center has been privileged to provide a medical home with quality comprehensive care to adults. Meeting the needs of each person regardless of their financial circumstances and helping to improve the availability and accessibility of essential primary and preventative healthcare is our constant goal.

Primary and chronic health care for adults provided by Rocking Horse includes scheduling for an Initial Evaluation, Follow-up Appointments, Diabetic Education Chronic Health Management, Specialty Visit Referrals, Medical Prescription Assistance Program and Mental Health Screenings.

Financial Assistance Available

In keeping with the mission of Rocking Horse Community Health Center to Raise Healthy Families in a Caring Community, our staff will assist you in securing any financial assistance you may qualify for. We have serveral assistance programs available, including but not limited to, our sliding fee and budget plans.

You must sign and date the bottom of the form and return it with a copy of your current household income. If you currently have no income, please provie a short statement of survival on how you are meeting your daily financial needs at this time.
